Adoption Agreement & Microchip Registration Form

Microchip: A microchip can’t do its job if your pet’s microchip is not kept up to date with current information. The microchip only contains an identification number; this number needs to be connected with your CURRENT contact information, so that your pet can be returned to you if he or she is ever lost. OAA typically uses Michelson Found Animals microchips. Found.org chips offer text/email alerts if your pet is located. You can enroll the chips in advanced microchip registries like Home Again if you wish.

Complete the following information, which we will use to initially register your pet’s microchip. You will receive a text and email to accept the pet when it’s registered. We will not – however – keep your pet’s microchip updated if you change addresses, phone numbers or emails. You must do this.

- I agree to keep said animal in my possession and to provide for its physical and emotional needs in such a manner as to meet the approval of the OAA. I also agree to abide by any laws governing said animal.
- The OAA has given this animal all required vaccinations up to the date of adoption. Adopter is responsible for any booster or annual vaccinations thereafter. Rabies vaccines are to be administered by a licensed veterinarian.
- In the event you can no longer care for your animal, you must return the animal to the OAA by calling the office (918) 742-3700 and making proper arrangements. Adoption records must accompany all returned animals. There no return reimbursement, unless approved by the president.
- The animal has been issued a rabies tag. It is state law that dog rabies vaccinations are to be up-to-date. OAA has ensured that at the time of adoption the dog’s rabies is current and will provide a tag and paperwork. Adopter is responsible for ensuring that the dog is maintains its legal status each year after adoption.
- OAA will only be responsible for veterinary care of the animal the first 24 hours after adoption for pre-existing conditions only. Identification of pre-existing conditions to be covered will be made by the OAA representative, and care must be provided by the OAA contacted veterinarian unless prior approval is obtained. Long-term veterinary care is the responsibility of the adopter, including treatment of pre-existing conditions.
- OAA does not guarantee or warrant the disposition, health or acts of the animal. OAA assumes no liability for personal injuries or damages to real or personal property caused by the said animal.
