Pet ID Tags

Oklahoma Alliance for Animals wants to ensure lost pets find their way back home – as soon as possible! If you need a free pet ID tag for your furry friend and live in Oklahoma, please complete the below order form.

Limit of 2 tags per family, please. Tags are mailed within 1 week of order placement.

WE WILL NOT MAIL TAGS OUTSIDE OF OKLAHOMA.

We have many tag shapes and colors; however our stock varies. Please choose your preferred shape and color and we will do our best to accommodate your request.

Pet ID tags are only effective when they are securely attached to a well-fitting collar. Make sure to keep your pet ID tag affixed to your dog or cat at all times.

*Pet ID tags are provided at no-cost. However, if you would like to make donation ($5 suggested) to cover the supplies and mailing fees associated with your request, you can do so by visiting our donation page.
